IMPORTANT:

Please note that the Delmarva Pain and Spine Center has specific network agreements with many insurance carriers and plans, but not all. Our In-Network and Out-of-Network status can potentially vary among the carriers based on the specific type of plan you may have. Your insurance policy is a contract between you and your insurance carrier. It is ultimately YOUR RESPONSIBILITY to contact the carrier to verify if our provider participates in your plan. If our provider is not In-Network, your plan may have Out-of-Network charges with higher deductibles and copayments, for which you will be responsible in whole. Any charges not paid by the insurance company will your responsibility. If you need any information to complete this verification, please feel free to call our office.

Lastly, if you are participating in an HMO plan or any other plan that may require a referral from a PCP, it is your responsibility to ensure that a referral is submitted to your insurance company. Our office can help verify if a referral has been submitted on your behalf. Note that without a referral, your insurance company will not cover charges for services rendered and you will hold complete financial responsibility.
